Legend Transitional Care Center is a 44-bed state of the art facility that was designed to meet the demands of short-term rehabilitation patients transitioning between acute care and home. We began admitting patients in January 2005.
Our primary focus is on patients who have had an acute care hospital stay and are still in need of skilled services prior to going home. This specialized unit consists of 44 private rooms equipped with piped gasses. It is complete with a fully-equipped therapy gym, a therapy kitchen and bath, as well as a spacious dining facility. Medicare, private pay and private insurance is accepted.
