Opened in May 2006, Legend Oaks Healthcare and Rehabilitation Center of Jacksonville is situated in a beautiful, quiet residential locale. We offer an open, spacious floor plan and a tranquil color scheme with several sitting areas for quiet conversation. Our residents also enjoy relaxing with the sounds of nature at our bird aviary and water fountain.

At Legend Oaks, our caring interdisciplinary team of nursing, social work, therapy, and administrative staff is what makes us special. Our experienced team of nursing professionals provide residential, skilled nursing, restorative nursing, hospice and respite care. We also provide intravenous (IV) therapy and specialized wound care. Licensed professional therapists provide physical, occupational and speech therapies onsite. In addition, our short-term rehab patients stay in our "state of the art" Rapid Recovery Unit. This unit is designed to help patients recover and return to health independence.

Legend Oaks offers a wide range of services and amenities. These include: a private dining room for special celebrations with family and friends, internet café for patients to send and receive emails and a beautiful courtyard, with covered patio to sit and enjoy a cup of coffee or read the newspaper in our "Texas sized rocking chairs."
